Here's How To Get Started Quickly

Call PerformCare at (877) 652‑7624

PerformCare is the contracted administrator for the services and supports offered through the NJ Children's System of Care partners, like Bergen's Promise. The referral process outlined below provides your child with the very best chance for a positive outcome. Please take a moment to familiarize yourself with these three critical steps for getting assistance for your child and family.

Step 1

Call PerformCare

Call PerformCare at (877) 652-7624. First, you will speak with a Member Services Specialist who will register the youth. Then you will be connected to a Care Coordinator (a NJ licensed clinician) who will gather information about the youth’s current behaviors and your reason for calling. This information will be utilized to determine the most appropriate clinical services. **Do not hesitate to ask your school representative, or your child's primary care, or mental health provider to assist you in making the call to PerformCare.

Step 2

Do Needs Assessment

The Care Coordinator may contact a licensed Clinician to complete a Needs Assessment to determine if you need more intensive support such as community-based care management services for your child.

(If your child requires immediate assistance to de-escalate challenging behaviors, your Care Coordinator may dispatch Children's Mobile Response and Stabilization Services.)

Step 3

Obtain a Referral

If your child has need for intensive wraparound care and support services and can benefit from a dedicated family and community team, you will then be referred to Bergen's Promise. This handout shows a pictorial version of this referral process.

We focus on prevention and early intervention by connecting a young person with the care they need in the most appropriate setting. Our goal is to help both families and caregivers create a more stable and healing environment for children, address barriers to well-being, and maximize youth and family strengths.
